Investigation Summary
On November 04, 2007 an employee was hooking up a trailer to a truck. He exited the vehicle after backing to the trailer and left the truck in reverse inadvertently. When the truck ball went into the hitch the truck moved backwards hitting the employee and breaking his leg. The employee was hospitalized and treated for a fractured leg.
