Employee Is Killed When Crushed by Falling Forklift

At approximately 5:15 p.m. on October 15, 2007, an employee loading the lift truck. He had backed the lift truck onto the trailer from the front to the rear. The lift truck width base was the same as the low boy trailer approximately 8 feet. The employee exited the lift truck from the operator's compartment and proceeded to walk around the back of the trailer. He proceeded to the driver's side of the trailer where the lift truck's right rear tire was slipping off of the trailer from a height of approximately 39 inches. The employee had a cell phone in his right hand when the lift truck fell from the trailer tipping over and landing on the employee pinning him under the right rear wheel, and crushing his head, left arm, and torso. The equipment involved was a Ingersoll-Rand telescoping reach lift truck, Model Number VR-1056C, Serial Number 173298, Unit Number 7050-0003. The height of the area above the rear tires of the low boy trailer was 39 inches. The height from the ground to the bottom of the rear axle of the lift truck was 18 inches. The width of the lift truck was 8 feet, and the width of the low boy trailer was 8 feet. The employee was killed.
