Investigation Summary
At approximately 1:34 p.m. on June 29, 2007, Employee #1, a service mechanic for an automobile dealer, was attempting to mount a tire to a rim. Employee #1 was applying pressure to a tire on a tire mounting machine, when the tire failed to seat. The tire mounting machine prevents tires from exceeding 50 psi, so Employee #1 took a nearby airline and continued airing the tire to 110 psi, when it exploded causing the bead to hit his arm and chest. The employee had been trained, but disregarded safety warning and company policy.
