Investigation Summary
On June 8, 2007, Employee #1 and three coworkers were framing a residential house, which was one of eight houses under construction in the area. The Foreman instructed Employee #1 and the coworkers to construct the second floor of the house by installing prefabricated floor joists. The joists were 12-inches wide by 14-feet long. Employee #1 was marking the areas on the top plate where the joists were to be installed (the layout), while the coworkers were cutting the joists to size. Both Employee #1 and the coworkers were using ladders to do this part of the job. After installing five or six joists, Employee #1 was standing on the top plate, which was 5.5-inches wide, while waiting for one of the coworkers to bring another joist in from outside for them to install. As Employee #1 had one foot on the top plate, he raised the other foot to rest on top of one of the joists. In doing so, he lost his balance and fell 11 feet to a concrete floor. Employee #1 was hospitalized and treated for a laceration above his left eye, which required stitches/ staples; a fracture under his left eye; a fractured left hand; and a head injury (hematoma).
