Investigation Summary

Investigation Nr: 202464707
Event: 05/02/2007
Employee Is Crushed Between Trailer and Tractor, Later Dies

At approximately 08:00 a.m. on May 2, 2007, Employee #1, a heavy equipment operator, was helping a coworker, the operator of a Row Crop 85 (four wheel drive tractor) to free a flatbed trailer stuck in the mud of a celery field. After successfully freeing the flatbed trailer, Employee #1 went to remove the chain tying the trailer to Caterpillar tractor. Employee #1 was between the trailer and the tractor at the time of the accident. The coworker shifted the Row Crop 85 into reverse and backed the vehicle and trailer towards the Caterpillar. The coworker assumed that Employee #1 was clear and did not check to see if Employee #1 was clear of the area. Employee #1 was pinned between the parked Caterpillar tractor and the trailer. Employee #1 sustained a fatal injury as a result of being pinned between a flatbed trailer and a Caterpillar tractor. Employee #1 was pronounced dead later in the hospital.
