Employee Fractures Arm Trying to Lower Van Lift Gate

On March 23, 2007, Employee #1 was working with four coworkers. They were loading boxes of merchandise from a storage building into the box van, which was equipped with a "Tommy Gate" lift gate. At the time of the accident, Employee #1 was located inside the van and attempted to lower the gate by reaching her right arm out and around to the control panel for the lift gate,. Employee #1's right forearm became caught in the lifting mechanism. Employee #1 received a fracture to her right forearm, which required surgery with one plate and six screws inserted. Employee #1 was hospitalized for four days with subsequent physical therapy.
