Employee's Fingers Are Amputated in Table Saw

At approximately 12:14 p.m. on December 18, 2006, Employee #1 was casting architectural ornamentations parts in the manufacturing area. He went to a table saw, Delta contractor's saw, Number 36-441, Serial Number 03C26040, to cut a piece of wood. The piece of wood kicked back and his left hand slipped into the blade. Employee #1 said the table saw had no guard or spreader. The general manager, project manger and the foreman said the guard, spreader and anti-kickback devices were always present and that Employee #1 was not authorized to use the table saw. Employee #1 amputated two fingers on his left hand and was hospitalized.
