Employee Is Killed When He Slips and Falls

At approximately 7:00 p.m. on December 9, 2006, Employee #1 was working as a laborer at a power plant which generates hydroelectric power. He was wearing his rain gear and boots. He descended a scaffold catwalk ladder onto the flat, dark, mossy surface covering a concrete floor to perform maintenance. He slipped on the mossy concrete floor as he was going to access the right side of the railing approximately 27.5 ft from the scaffold catwalk ladder. He slipped again and fell down a 1 ft rise to a 3 ft run sloped discharge ramp apron approximately 71 ft in length. He went underneath a railing set up horizontally across the apron and fell over the edge of the ramp apron 37 ft. Employee #1 landed on the concrete tail bay floor, where he suffered a concussion and died.
