Investigation Summary
At approximately 6:30 a.m. on July 27, 2006, Employee #1 was retrieving paper from a manual paper cutter, when the arm of the paper cutter came down and lacerated his fingers, requiring surgery. He was in the hospital less than twenty-four hours. The handle was not locked in place prior to removing the paper. The employer provided documentation and a supervisor statement that the employee was properly trained and acted independently. The employer had an effective IIPP in place. No citations were issued.
