Investigation Summary
At approximately 13:30 p.m. on August 23, 2006, Employee #1, a carpenter, was cleaning construction debris under unprotected falsework for a general engineering contractor that conducts construction of highways, bridges and other infrastructures. As he was removing the debris, the falsework that he was under collapsed. Employee #1 was taken to St. John's Regional Medical Center, where he was hospitalized and treated for multiple fractures.
