Investigation Summary
On May 22, 2006, Employee #1 was standing on an 18 ft tall metal ladder, nailing wall panels together inside a home under construction. He was using both hands to take a nail out of the panel when he lost his balance and fell backward, landing on the plywood floor. Employee #1 was transported by coworkers to Fauquier Hospital, where it was determined that he had suffered serious head injuries. He was immediately flown to Fairfax Hospital, where he died on the morning of May 24, 2006.
