Employee Dies in Fall from the Truck

At approximately 8:50 a.m. on October 26, 2006, an employee was unloading a plastic tote from the bed of a truck. The employee was holding onto the top of the tote and pulling it across the bed of the truck so he could load it onto a forklift. The hinged door that the employee was holding gave way. This caused him to lose his balance and fall to the concrete, four feet below. The employee died the same day at approximately 7:00 p.m.
