Employee's Arms Are Amputated in Shredder Machine

On January 23, 2007, Employee #1 and a coworker were cleaning drywall waste from a drywall shredder machine without the guard in place and without locking and tagging out the machine. Employee #1's screwdriver fell into the rotating knives. When he reached for the screwdriver, his hand was pulled into the point of operation. While trying to remove his arm, his other hand was pulled into the machine. Both of the Employee #1's arms were amputated between the shoulder and the elbow before the emergency stop could be pressed by the other employee. Employee #1 was hospitalized however neither of his arms was salvageable.
