Employee Falls Into Molten Metal and Is Killed

Employee #1 was controlling the pouring of molten metal into a vertical 30-ft cylindrical mold by way of a hand-cranked ladle when he fell off the inadequately protected portable platform and stairway. Employee #1 fell approximately 35 ft into the holding pit which became engulfed with molten metal moments later due to a run-out through a vent hole in the mold. Three other employees began the descent into the pit but were turned away by the run-out's 2,600-degree Fahrenheit temperatures. If the run-out occurred as little as 30 seconds later than it did, this case would likely have resulted in four deaths. Employee #1 was killed.
