Worker Smashes and Fractures Finger with Dolly

At approximately11:15 a.m., on January 29, 2011, Employee #1, a 24-year-old male with Fed Ex, went to the yard to check the trailers to ensure nothing was left inside the parked trailers. Employee #1 noticed a dolly was parked improperly behind a trailer he was going to inspect. Employee #1 observed that dolly was parked in an angle and blocking his access to reach the back-door of this trailer. One wheel of the dolly was parked up on the curb (approximately seven inches off the ground) and other wheel was at ground level. As the Employee #1 leaned over the dolly to reach this trailer's back door to open, the dolly rolled off the curb and smashed his fingers between the bumper of the trailer and the dolly's eye-hook. Employee # 1 fractured his index and middle fingers on his left hand. Employee # 1 was hospitalized more than twenty-four hours from the event. During the course of this investigation, the FedEx management staff (Senior Manager, Safety Specialist), Employee #1 and an Independent contractor (FedEx Truck Driver) were interviewed and equipment (Dolly involved in the accident), documents, statements and pictures were also reviewed.
