Investigation Summary
At approximately 9:15 a.m. on January 10, 2011, Employee #1 was working on a roof at a renovation project at a Safeway Store located in Daly City, California. Employee #1 was an employee of Roof Guard Company, Inc., a subcontractor at the Safeway Remodel Project. Prior to the accident, Employee #1, along with two coworkers, was walking on the roof picking up residual debris from the previous weeks' roof removal, in preparation for the removal of the plywood sheeting. Employee #1 was wearing a harness, but was not tied to a rope since he was 10-14 feet away from the edge of the roof and had not yet started the process of removing the plywood on the roof. Employee #1 was walking, and he stepped on weak plywood. Employee #1 fell through the roof and landed on a forklift overhead guard located below, and then fell down to the concrete floor and fractured his back. Employee #1 fell from a height of 22 feet, 9 inches. Emergency services were contacted and responded to the site. Employee #1 was taken to San Francisco General Hospital for treatment, where he was hospitalized for three days.
