Investigation Summary

Investigation Nr: 201127701
Event: 09/01/2010
Employee Fractures Fingers When Entangled in Machine

At approximately 5:00 a.m. on September 1, 2010, Employee #1 was operating a Salami Brushing machine made by R.S Randall & Co Engineering Services. The machine had 8 metal shafts with brushes, which rotate and remove mold from the salami. Once the machine is turned on, the shafts elevate up 50 inches to reach the limit switch. After touching the limit switch, the shafts rotate and travel downwards to the floor. The distance between the rotating shafts and the platform was 8 inches. Employee #1 was standing in the chamber that was 3-ft by 7-ft to check the brushes to see if they were dry. The open chamber was unguarded and he turned the machine on to rotate the shafts. As the shafts rotated and travelled halfway down, he inserted his right hand near the shafts to check for a mist on his wrist. While Employee #1 moved his hand above the shafts, his glove caught in the first shaft on his right side. He was unable to reach the 'Stop' switch, but after a struggle, he pulled his hand out. He sustained two fractured fingers and torn ligaments. Employee #1 was hospitalized for more than 24 hours and surgery was performed on his hand. He stated that he was never trained on how to check the brushes to see if they were dry.
