FALL FROM LADDER

On February 12, 2010 at approximately 11:00 AM, Employee #1, a supervisor at Chuck E. Cheese Restaurant was injured while trying to reconnect a power cord for a food-warming lamp mounted onto a food preparation counter. The electrical outlet for the device was a ceiling mount fixture that had become worn. It would no longer hold the cord firmly, resulting in intermittent periods of power loss to the warming lamp. Employee #1 used a portable ladder/step-stool with two steps to access the counter height of about 35" from the floor. After reconnecting the power cord while standing on the table, Employee #1 fell while trying to step down onto the upper ladder step. Employee #1's right foot became caught on the handle at the top of the step-stool, which caused her to lose balance, falling onto the tile floor, landing on her back and striking her head. Employee #1 was taken to San Mateo General Hospital and released about 5:00 PM. Employee #1 incurred no fractures, and returned to work on restricted duty after 6 days off.
