Investigation Summary
At approximately 12:30 p.m. on January 7, 2010, Employee #1 was installing gutters. He was approximately 10 ft high on a 24 ft extension ladder, an extra heavy duty, Type 1A, maximum working length of 21 ft, duty rating of 300 pounds, Model Number 610214, manufactured by Green Bull, Inc. The ladder was sitting on the ground, resting on a fascia board and was not tied. Employee #1 was reaching over to his right, to connect a 24-in. prefabricated gutter, and fell. He landed on his right side, fracturing his pelvis, leg and ribs. He also suffered a compression fracture of the spine and was hospitalized.
