Employee Falls from Platform and Injures Back

At approximately 1:55 PM on September 23, 2009, Employee #1 was walking by the forms (wood wall) when his nail-bag was caught in a pin (tension between the forms). When Employee # 1 yanked his body to free the nail-bag, he lost his balance and fell into a creek, which was approximately four feet deep. Employee #1 injured his back. He was taken to the San Francisco General Hospital, and was released the next day. After this incident, the general contractor installed a guard rail on the creek to prevent future accidents. The San Mateo District Office of DOSH was notified of the accident. Initially, the Fire Department reported this accident. The next day, the employer (Aparicio Cement contractors) followed up with more details. The initial investigation was initiated on October 15, 2009.
