Employee Is Injured in Fall from Theatre Stage

Approximately between 10:00 a.m. and 10:30 a.m., on May 29, 2009, Employee #1 was instructed by her supervisor to install reflective duct tape at the edges of a large theatre for a high school. Employee #1 entered the theatre, which was unlit with the exception for exit signs on top of the doors. As Employee #1 walked up to the right side of the theater stage and crossing to the opposite side to turn on the lights, she fell approximately 7.92 feet to the concrete orchestra pit. Employee #1 was hospitalized, where she was treated for her injury. The accident investigation revealed that the stage area only had 1 foot-candle of light present from the exit lights. The employer failed to provide assembly areas, working areas, stairways, aisles, and passageways with adequate natural or artificial illumination.
