Employee Is Killed When Crushed in Screw Conveyor

At approximately 6:30 p.m. on June 4, 2008, Employee #1 and two coworkers prepared and started work manufacturing a large order of asphalt for a pavement job. Approximately 30 minutes later, the process stopped due to the disengagement of the shear bolt shift connection from the pug mill screw conveyor and the main slat conveyor. They started repairing the shear bolt shift connection from the pug mill screw conveyor to the main slat conveyor without implementing a lockout program. Employee #1 was inside the auger reattaching the pug mill screw conveyor shear bolt connection. One coworker was readjusting the belt and pulley system in order to align the holes for the shear bolt pin, and one coworker gathered the necessary parts and tools and provided them to Employee #1. While a coworker was about to hand over a box wrench to tighten the shear pin bolt connection, he dropped the box wrench into the main slat conveyor. The coworker went to the control room in order to turn off the main slat conveyor without informing Employee #1. The coworker accidently pressed the start button of the pug mill screw conveyor while Employee #1 was still inside the auger. Employee #1 was killed.
