Employee's Hand Is Injured in In-Running Conveyor Nip Points

At approximately 10:10 a.m. on August 12, 2007, Employee #1 was working in the laundry room of a hotel. She was using a laundry folding machine (Model Number S-12-2000, Serial Number 47448), when she tried to untangle the conveyor ribbons. Without first deenergizing the machine, she went around the side stacker unit, sat down on the floor and then laid down below a section of the folder to fix the ribbon. She reached her hand underneath and beyond the machine guard to access the roller and ribbon area. The employee had a coworker turn and the machine "off" and "on" as she used a ball point pen to realign the ribbon. Once she fixed it, the coworker joined the employee in the area under the machine, where she demonstrated how she fixed it. As she pointed, her fingers on her left hand were caught in the in-running nip point of two of the drive rollers for the ribbons. The machine was running at a normal operating speed and drew in her hand palm-downwards up to about mid-palm. The coworker shut down the machine using an emergency stop button, and the employee's hand was fractured, bruised and cut. Employee #1 was transported to Stanford Hospital, where she was treated for left hand injuries but not hospitalized. The employee was not authorized to perform the service procedure but stated that management was aware that the crossed ribbon problem was fixed by unauthorized employees in the past. She also stated that she had seen other laundry employees and engineering staff perform this procedure before without deenergizing the machine. The employer was cited under T8 CCR 3314(g)(2)(A) The Control of Hazardous Energy for the Cleaning, Repairing, Servicing, Setting-Up, and Adjusting Operations of Prime Movers, Machinery and Equipment, Including Lockout/Tagout; T8 CCR 4184.
