Investigation Summary
At approximately 2:30 p.m. on March 4, 2007, Employee #1, a new hire of two weeks, was standing on the lift gate of the truck to unload a pallet of groceries, which was placed on a pallet jack. The pallet was 4-ft by 3.5-ft by 6.5-ft. When Employee #1 noticed that the load on the pallet was shifting and leaning to the side, he attempted to stabilize and secure the load. The load tilted on him and made him fall to the ground. His head and body hit the ground and his foot and ankle became wrapped around the metal chain hooked to the lift gate of the truck. Employee #1 fractured his right leg, which required surgery. He was hospitalized for five days.
