Investigation Summary
At approximately 11:00 a.m. on January 18, 2007, Employee #1 was reaching into a card box to grab a piece of paper when he lost his balance and the upper part of his body (up to waist) got stuck in the compactor. The compactor was not secured or restrained from moving. When Employee #1 struggled to get out, the approximately 1-cubic-yd compactor fell over him. One upper-front corner of the compactor was lodged into the sheetrock wall and kept the compactor from falling all the way to the ground and crushing Employee #1 completely.
