Employee's Three Fingers Are Amputated in Brake Press

On July 12, 2007, Employee #1 and a coworker were working on a brake press in the West Bay at Paxton and Vierling Steel, Incorporated. in Carter Lake, Iowa. They were bending parts. The coworker was operating the pedal, and Employee #1 was helping set the parts and removing the parts after being bent. After the part was bent, Employee #1 reached into the die to grab the part, while the machine was still being cycled through. Employee #1's hand was struck and smashed, and his left hand middle three fingers were amputated. He was hospitalized.
