Employee Toes Amputated When Caught by Conveyor

On June 25, 2007, Employee #1 caught his left foot between a vibrating conveyor and a paddle machine. A board was stuck in a conveyor and Employee #1 came down a discard chute and tried to dislodge the board. His left foot slipped and lodged under the paddle machine. The machine tore the steel toe part off of the boot, exposing the toes to a hammering action and causing the injuries and partial amputation of the second and third toes.
