Investigation Summary
At approximately 10:00 a.m. on February 22, 2007, Employee #1 heard a clicking noise on a Chore-Time conveyor belt transfer system. Employee #1 reached into the machine to determine the mechanical problem. The machine had not been disconnected from the power source, nor had the power been shut down. The guards were designed to be easily removed. Employee #1's left hand was caught between the roller and belt. Part of Employee #1's left hand was severed by the machine. Employee #1 was taken to the hospital where the remainder of the left hand was removed at the wrist. Employee #1 was hospitalized.
