Investigation Summary
On January 2, 2007, an employee was working on a platform that was hoisted 9-ft above the ground by a powered industrial truck, fork truck. The platform was not guarded nor was the employee furnished with any fall protection. The employee fell from the platform, striking his head on the concrete. The employee was killed.
