Employee Amputates Finger While Operating Rolling Machine

On September 9, 2006, Employee #1 was using a Victaulic groove rolling machine, Model Number HM-262, to groove galvanized pipe that was 6 inches in diameter and approximately 37.5 inches long. After he completed the groove on one end of the pipe and began to rotate the pipe in preparation for grooving the other end, the machine power source engaged. This caused the pipe to rotate clockwise, amputating Employee # 1's left index finger above the first digit.
