Employee Is Killed When Run over by Forklift

On June 19, 2006, Employee #1 and a coworker were maneuvering a malfunctioning forklift with an operating forklift to an area where it would be repaired. While Employee #1 walked around the back of the operating forklift after setting a tool down, the forklift made a sharp left turn. A rear wheel of the forklift ran over Employee #1 and killed him.
