Investigation Summary
On February 22, 2008, Employee #1 was troubleshooting the forward mode DC circuit of a forklift. He was kneeling with his left leg on the ground and his right leg was at 90 degrees to grade. With this posture, he pressed the directional contactor with the left hand and the L1 A contactor with the right hand using appropriate tools. The L1 A contactor got stuck and the forklift jerked forward, travelling a distance of approximately 12 inches, striking the Employee #1 and pinning his left foot under the rear right tire. He sustained lacerations and a fracture of the big left toe. Employee #1 was admitted to Washington Hospital on February 22, 2008, treated and discharged on March 20, 2008. Based upon the interviews conducted and documentation developed during the course of this investigation, Employee #1 did not follow the appropriate procedure for electrical circuit troubleshooting. He was trained and experienced.
