Investigation Summary
At 12:40 p.m.on September 24, 2007, Employee #1 was walking in the hallway of the main floor of the Berkeley Arts Magnet at Whitter, when she slipped on a pool of water near a drinking fountain injuring herself. Employee #1 uses a cane to assist in her mobility and was on her lunch break when the accident occurred. There was no one in the area at the time of the incident and Employee #1 was discovered by a student whom reported the accident to the main office. There were no witnesses to the incident except for Employee #1. Based upon the interview conducted with the employee and documentation developed during the course of this investigation, the cause of the accident was water puddles on the floor near the drinking fountain.
