Investigation Summary

Investigation Nr: 201507704
Event: 06/22/2007
Employee Cut Hand on Table Saw

On June 22, 2007, Employee #1, a foreman/millman was adjusting the fence on an Oliver Machinery Model 232-D 10-inch table saw to cut a 2 inch by 1 inch by 1 inch by 19 inch block. He had the blade running and had the block material at the front of the blade, not the back of the blade where it is normally fed through. While Employee #1 made the adjustment to the fence with his right hand, the block caught the blade and kicked back, bringing his left hand holding the block across the blade, with the block hitting his right hand. Employee #1 suffered severe lacerations to his thumb and index finger on the left hand, and a dislocated middle finger on his right hand. He was taken to Kaiser Hospital in Oakland where he was treated and had surgery, then released the next day.
