Employee Is Injured When Trapped in Conveyor Belt

At approximately 1:00 p.m. on April 3, 2007, an employee, a Sand Muller Operator, was getting ready to shut down operations when he got a call from the Molders for more sand. He started the system, but forgot to start the distribution belt; therefore, sand was building up and started to fall from the incline end. He was told by Coworker #1, the Maintenance Supervisor, to start up the distribution belt. The employee started the belt and then went up to the catwalk to clean up the area where the sand had fallen. Coworker #3, a Maintenance Mechanic, rushed in to tell Coworker #1 to shut off the machine and belt. He saw the employee stuck in between the distribution conveyor belt. The tail end of the conveyor was not guarded nor did the employee lock out the conveyor prior to cleaning the area. The employee suffered a fractured right arm, clavicle, ribs, vertebrae, lacerated liver, punctured lungs, and head contusion. He was transported to Eden Medical Center in Castro Valley, where he was hospitalized for three weeks.
