Employee Falls from Forklift and Sustains Skull Fracture

On February 25, 2007, Employee #1 was in aisle Number 85 of the warehouse, working on a forklift stacker. Employee #1 was on the stacker to pull furniture for a customer order. Employee #1 failed to tie off his harness to the stacker. Employee #1 fell approximately 14 ft from a forklift stacker and suffered a skull fracture and internal bleeding of the head. Employee #1 was hospitalized for three days and returned to the hospital for further evaluation.
