Investigation Summary

Investigation Nr: 201507043
Event: 07/25/2006
Employee's Fingertip Is Amputated While Operating Die Unit

On July 25, 2006, Employee #1, a machine operator, braced his left hand on the machine, a William C. Staley Printer-Slotter-Creaser-Rotary Die unit. He was operating the key switch with his right hand, located 69 inches above the floor. The bracing with the left hand was required due to the machine configuration that juts out from floor to waist level, requiring awkward postures to reach the control panel, which lies above and behind the machinery jutting out below. The natural hand position, when bracing with the left hand, is very close to the nip point created when the sections of the printing machine close together. Employee #1 was closing the sections, when his left middle finger entered the nip point and was crushed as the sections closed together, and amputated his fingertip. He was hospitalized as a result of his injuries.
