Employee Is Injured When Slag Falls

On July 23, 2006, Employee #1 was working as a cupola prep charger. He had been cleaning slag from the walls of the cupola with a jackhammer. The employer had not identified nor evaluated the hazard of the slag vibrating and falling off the walls of the cupola. When another worker was using the jackhammer, a piece of slag that weighed approximately 800 lbs was vibrated loose. Employee #1 was injured when the piece of slag fell, struck him and pinned his left leg to a piece of slag that was already on the floor. He was hospitalized for unspecified fractures.
