Investigation Summary

Investigation Nr: 201506896
Event: 06/12/2006
Employee Is Injured When Leg Becomes Caught in Driveshaft

On July 12, 2006, Employee #1 was working alone inside the fenced-in property of his employer. Within the fenced-in property was a slurry pond that the employer used to distribute water to other areas throughout the property for recycling. To distribute this water, a 10-horsepower electric motor powered a water pump that was connected by a rotating driveshaft, which had universal joints at each end. As Employee #1 was starting the slurry pond pump by maneuvering a small valve, he stepped over the rotating driveshaft, which caught the cuff of his coveralls. Employee #1's leg was pulled into the blades, and was seriously injured. Employee #1 was hospitalized and treated for the amputation of eighty percent of his leg.
