Employee Is Killed When Struck by Falling Block Wall

Employee #1 was on break with a coworker waiting for two other coworkers to exit the telescoping man lift basket. The two coworkers were removing a 20-ft concrete block wall with a hammer drill to the 12-ft level. The wall had a lateral section of slab floor 3 ft above grade that was undermined. When Employee #1 and his accompanying coworker were entering the basket, the wall, being shorter than before, did not apply enough downward force to keep the counter-weighted slab level. As a result the slab tipped with the 12 ft high concrete block wall section falling toward Employee #1 and the coworker. The wall struck the coworker knocking him away. Employee #1 was pinned between the block wall and the welded frame ceiling shoring scaffold system that was approximately 6 ft away. Employee #1 was declared dead at the scene. The walls and ceiling were rebraced before recovery operations could begin. The original narrative made no mention of the coworker's injuries.
