Investigation Summary
The job site foreman directed two employees to go to the second floor of a single family home they were framing to arrange 48-in. by 96-in. by 0.75-in. thick plywood sheets on the floor, completing the second floor and covering all the holes. Employee #1 was already on the second floor cutting wood sheets. Employees #1 and one of his coworkers worked together to cover the floor joists over the smaller area, approximately 64.5-in. wide by 10-ft to12 ft long. One piece of plywood was in place over the joists. The second plywood sheet was already cut to approximately 84-in. long. This had been placed over the space created with the joists. Employee #1 suggested the coworker stay on top of the first plywood sheet, adjusting the end of it, while Employee #1 walked over to the end of the second sheet to adjust it at that end. The end of the second sheet was not supported and, as Employee #1 stepped on it, the sheet pivoted on a joist, allowing Employee #1 to fall approximately 10-ft 10-in. through the joists to the concrete floor. The joists were set at 24-in. on center and, when Employee #1 fell through the joists, he hit an adjacent joist which broke free and fell to the floor. Both the joist and sheet of plywood fell to the first floor. Employee #1 sustained fatal head injuries.
