Employee Is Struck by Forklift and Later Dies

On June 29, 2006, Employee #1, a delivery driver, traveled under a load and between a powered industrial fork truck mast and the flat bed of the tractor trailer. The fork truck driver was signing documents and was not looking when his foot left the brake and the fork truck traveled forward pinning Employee #1 between the fork truck and the flat bed. Employee #1 died later as a result of his injuries.
