Employee Is Killed When Struck by Reversing Forklift

On July 31, 2006, Employee #1 was a member of a crew who was collecting chickens in a chicken house or barn at a farm. The chickens were caught by hand, and placed in cages. At the same time, another employee from the Gold Kist, Incorporated, was operating a forklift, Manitou K-D Hawk, Model Number M30-4H, Serial Number 751383, to move the chicken cages. The forklift had warning lights on its front and rear and had an operable back-up alarm. Employee #1 and other crew members wore reflective clothing while collecting the chickens. Employee #1 was trying to catch a stray chicken, and walked behind the reversing forklift. Employee #1 was struck by the forklift and was killed.
