Event: 05/04/2006
Employee Is Killed from Electrocution after Contacting Lines

At approximately 11:00 a.m. on May 4, 2006, Employee #1 was in the process of power washing the garage. He I was found lying on the ground next to the garage after being electrocuted. The investigation revealed that the employee placed the 20 foot ridge ladder over the electrical lines that were approximately 9 ft above the roof's peak. The electrical lines were carrying 4,250 volts. The investigation also showed that the employee was standing on a 12 by12 pitched roof approximately 10 ft high when he fell after coming into contact with the lines. The employee was using a 3504 Contractor Series power washer by Sherwin Williams. The jobsite was damp as it appeared to have rained earlier in the day. The temperature was 63 degrees. No other employees were on the job as he was working alone. The investigation also showed no job specific training had been provided to the employee on electrical hazards.
