Employee Is Killed When Crushed by a Pallet of Paper

On April 20, 2006, Employee #1 was operating a Crown forklift, without the overhead guard in place. He was moving two pallets of paper, one on top of the other. Each weighed 1,200 lbs. Employee #1 was killed when the forks hit the wall and the top pallet of paper fell on him.
