Employee Dies in Fall from Ladder

On July 27, 2006, Employee #1 was in the process of cutting a metal chimney approximately 7-ft high, while standing on an approximately 6-ft ladder. Employee #1 fell approximately twenty-seven feet down to the next floor level (8th floor). Employee #1 sustained multiple bone fractures and died at the hospital on August 19, 2006.
