Carpenter Is Killed When Work Platform Collapses

On May 3, 2006, Employee #1 and a coworker were erecting a portico addition over the entrance to a four-story garden apartment building. They had rigged a makeshift work platform by attaching the perimeter boards to the columns of the portico, and then laying decking across the perimeter boards. There had been two previous similar portico erections prior to the date of the incident. After erecting the columns, the men assembled a deck system by laying boards over the perimeter rail joists. The deck boards overhung the perimeter boards. There was no guard railing installed and no fall protection equipment in use at the time. While both workers were on one end of the platform, and Employee #1 was standing on a step ladder placed on the platform deck, the scaffold failed, and both workers fell to ground. Employee #1 sustained head injuries and was killed. The coworker was hospitalized for leg and back strains.
