Investigation Summary
At approximately 7:15 a.m. on March 22, 2006, Employee #1, a laborer, was working in conjunction with a lull operator who was unloading bundles of perlins from the trailer. This was the first day that the crew was unloading perlins. Employee #1 was positioned on top of the trailer, acting as the spotter for the operator and rigger, attaching the bundles to the forks of the lull. During this unloading process, Employee #1 motioned the operator to raise the load. When doing so, he moved towards the center of the lift. The bundle started to fall and as it did, one end rose up hitting Employee #1 in the face and knocking him off the trailer bed. The fourth perlin being lifted fell off of the forks, hitting Employee #1's head while he lay on the ground. He died at the scene.
