Investigation Summary
At approximately 9:30 a.m. on February 3, 2009, Employee #1, a mechanic, was working on the underbelly of a R44 helicopter, installing cabin heater parts. Employee #1 sustained a serious injury to his left eye and was taken to the hospital, where he underwent surgery on the following day to have an antenna removed from the lower eyelid of his left eye.
