Investigation Summary

Investigation Nr: 202529186
Event: 01/20/2009
Employee Is Injured in Fall from Scissor Lift

At 12:20 p.m. on January 20, 2009, Employee #1, an electrician working for ECU Incorporated, was working from a scissors lift installing electrical conduit near the second level of the customers' Customer Bag Line, the primary intake conveyor line where dirty linen is received for cleaning. Employee #1 could not reach the conduit to connect some couplings. Rather than reposition the scissors lift, he stepped out of the scissors lift without his full body harness and walked onto the arms and mast of an inactive laundry lift near the couplings. The arms were being kept in position, until further construction, by a brace/aligning bracket tack welded between the arms. Employee #1 stood on this brace and it broke, dropping him 11 ft to the pavement. Employee #1 struck his head and was taken by ambulance to Long Beach Memorial Hospital, where he was admitted and treated for a fracture and concussion.
